Customer Service

At Vita Health, we are committed to helping you and your family get healthy and stay healthy. Please contact Customer Service if you need to do any of the following:

  • Ask for assistance in the application process
  • Choose or change your PCP
  • Ask about benefits and services
  • Get help with referrals
  • Replace a lost identification card
  • Ask about any change that might affect you or your family's benefits
  • Other member question or concerns

Unlike state administered health plans, Vita Health employs its own enrollment and customer service representatives that offer extended hours Ð until 7pm, Monday-Friday. An on-call nurse is also available.

For employers, representatives are available to begin offering Vita Health to employees.
