•	You meet our income criteria*
•	You have not had health coverage for the past 6 months and do not qualify for any public health coverage or public health programs


Our Vita Health members are only responsible for a share of the monthly premiums. Co-payments are due at the time when health care services are obtained. Our premiums are affordable and range from $25 to less than $100! The additional share of your premium is subsidized by the Health Care District of Palm Beach County.
